Our Minnesota services are focused on supporting the unique goals, strengths, and needs of each individual while helping families navigate every stage of the journey.
Our Residential and In Home Support services help individuals build independence, develop daily living skills, strengthen community connections, and achieve personal goals while receiving the level of support that is right for them.
Many disability services in Minnesota are funded through Minnesota Health Care Programs (MHCP) and administered through county agencies and managed care organizations.
Before services can begin, the lead agency (County or Tribe) must authorize services through Minnesota's MN-ITS system.
Authorization is based on assessed needs, eligibility requirements, and approved service plans.
Minnesota utilizes a fee-for-service reimbursement model.
Services are documented according to established billing units, which may include:
Authorized services are billed electronically through the MN-ITS portal and reimbursed by the State of Minnesota or the applicable Managed Care Organization (MCO).
This process helps ensure services are delivered and reimbursed in accordance with Minnesota Department of Human Services (DHS) requirements and 245D regulations.
